Civil Code (Cap. 16), article 301
301. The officer mentioned in article 296, howsoever he may
have received information of the death of any person, shall, af ter
ascertaining such death, draw up , within two days from the rece ipt
of such information, an act according to Form G in Part II of t he
First Schedule to this Code containing the following particular s:
(a) the date of the act;
(b) the name, surname, a legally valid identification
document, age, place of birth, and place of residence of
the deceased;

(c) the name and surname of each of the parents of the
deceased stating whether th ey are alive or dead;

(d) the name, surname of the man or woman, if the dead
person was married, in a civil union, i n a c o h a b i t a t i o n
registered under the Cohabitation Act or enrolled by
means of a public deed under the Cohabitation Act, 2020
or a widow or widower;
(e) the hour, day, month, and year when, and the place
where the death occurred , and the place where the
deceased was, or w ill be, buried;
(f) the cause of death.
