Code of Organization and Civil Procedure (Cap. 12), article 519D
519D. (1) A demand for the appointment of a guardian shall be
made by application to the Guardianship Board in accordance wit h
article 188A of the Civil Code .
(2) The application shall contain a statement of the facts on
which the demand is founded and an indication of the witnesses, if
any to such facts.
(3) Any documents in support of the demand shall be filed
together with the application.
